Lifestyle Consultation with Exam –
Functional Medicine

We take wellness seriously. Patients come in with many different complaints. The question we help answer is: What is the Underlying Cause? In order to help, we must find this underlying cause.

Dr. Johnson will search for the underlying causes that contribute to your illness through cutting edge lab technology and tailor the recommendations to your specific and individual needs.

We also understand that outlining what you can expect will decrease some of the initial apprehension and get you on the road to improved health much faster.
History
The most important part of any medical work-up is a thorough history. We have carefully developed a questionnaire that will allow us to obtain the most comprehensive and detailed history possible.
Like a good CSI agent, uncovering the underlying factors that have caused or are causing your present health problem(s) is essential for getting well. That will all start with our medical review of your records. It is your responsibility to request your medical records for the last three (3) years from all of your physicians and any lab work and have them forwarded to the office.
Medical Records
Once we have all Medical records from the last 3 years, we will inform you of the Charges for the Consultation. This includes the questionnaire, the 60 minute consultation, examination, and the time for the doctor to thoroughly study your medical records. Once received, we will email you the link to the questionnaire. This way you will know up front what the charges are.
First Consultation – The cost for your first consultation is $250.00-$450.00

Fast, effective pain relief of injuries using up-to-date technology and old school customer service

On your first visit with Dr. Johnson, she will have completed her review of your medical questionnaire and reviewed medical records from your physicians. This will commonly provide significant information for her to determine what to do next to help you get well.
Detailed Report
You will receive a detailed report from the doctor summarizing her findings and letting you know the possible underlying causes/reasons for your health challenge. The next step may require further testing consisting of physical examination, lab testing and other medical diagnostic testing.
Lab Work
Our main objective is to carefully identify what has possibility been missed by other physicians and to give you some logical direction to help turn your health around.
Lab fees for the above services will be discussed at your first consultation. Once Dr. Johnson has the results of the above testing, she will be able to outline the most effective treatment plan.
Advanced Diagnostic Testing
This further testing could include: physical examination, advanced diagnostic testing, genetics or a combination of these. The physical evaluation may consist of orthopedic tests, muscle tests and alternative methods to see how the body is dealing with stress and imbalances. Dr. Johnson will also evaluate the underlying physiology of the dysfunction. She will provide you with the best wellness plan for your particular body and stressors. This wellness plan will help with conditions in 2 steps:
1. Add what’s lacking in the body to nudge its physiology back to a state of optimal functioning; and
2. Remove anything that impedes the body from moving toward this optimal state of physiology.
Through education, diet, and supplements, you can obtain the results you want and get closer to optimal health.
